2. Coordination of Supports (Level 2)
We coordinate multiple supports, assist you to navigate providers, and help you overcome challenges or barriers.
We focus on building your capacity, skills and confidence to manage your plan independently.
3. Specialist Support Coordination (Level 3)
For participants with complex needs or high-risk situations.
Our Specialist Support Coordinators work with you and your team to design clear, practical strategies that keep your supports stable, safe and effective.
